一些临床前研究和临床研究已经证实了脑血管在颅脑创伤相关功能障碍中的重要性 。Veksler及其团队[16]使用改良的核磁共振动态对比增强扫描技术(DCE-MRI)方案,证明反复暴露于头部撞击的美式足球运动员存在慢相血脑屏障障碍,这反映了微血管功能障碍 。而在参加非接触式运动的运动员(年龄和性别匹配)中未观察到类似变化 。尽管每个个体损伤的空间位置不同,但这种血脑屏障破坏可持续数月之久,且与弥散张量成像显示的轴索损伤有关 。此外,研究人员在两个具有相关神经病理变化的啮齿动物模型中重现了这些结果,表明颅脑创伤相关的微血管功能障碍十分常见,且可以使用DCE-MRI这样一种基于现有的核磁共振扫描机器和软件进行设置相对简单的成像序列进行检测 。血脑屏障破坏也是一种有希望的药物干预候选靶点 。
